Enclose the orignal dated register receipt, with purchase price and date circled an the UPC Symbol/Bar Code from the Philips energy saving light bulbs you purchased, with the completed rebate form.
There is a limit of 1 rebate per household, family, group or orginization. Purchases need to be made between 9/1 and 11/15/10.
